"EXPRESS HOW UC SUCCESS" CONTEST
OFFICIAL RULES
NO PURCHASE NECESSARY
1. HOW IT WORKS:
Celebrating UC Success is designed to recognize people whose lives are impacted by ulcerative colitis (UC). This year's program encourages UC patients to creatively express their personal experiences as they pertain to overcoming the challenges of their condition. We encourage patients to describe how this creative expression has influenced and/or impacted their lives. We are asking you, the UC patient, to "Express How UC Success."
2. WHAT TO DO:
You must submit the following two elements ("A" and "B"):
A. A representation of how you creatively express yourself through an activity such as art, music, dance, gardening, decorating or any other activity that could be described as being a "creative expression."

Submit ONE of the following:
- A photograph that depicts your creative expression
(for example, a photo of your original artwork, gardening, etc.), OR - a soundtrack of your creative expression
(for example, a soundtrack of your original song, speech, etc.), OR - a video of your creative expression
(for example, a video of your original dance, performance art, etc.), OR - a "written work" (500 words maximum)
(for example, your original poetry, short story, etc.)
B. A 1500-character (maximum, including spaces and punctuation) original "written statement" that describes how this form of expression has influenced and/or impacted your life.
3. TWO WAYS TO ENTER:
- ONLINE: Beginning at 12:00pm (noon) (ET) on 2/10/10 through 11:59:59pm (ET) on 4/21/10, visit www.ucsuccess.org OR www.facebook.com/ucsuccess, and follow the online entry instructions, uploading and file/format requirements. Any use of robotic, automatic, programmed or the like methods of participation will void all such entries/submissions. Limit one online entry/submission per email address/user id per day.
- VIA MAIL: On an Official Entry Form only (available to download and print at www.ucsuccess.org), type or clearly hand print your complete name, birthdate, street address (no P.O. Boxes), city, state, zip code, daytime telephone number, (optional) evening telephone number, and (optional) email address. Be sure to check off the applicable box to indicate if you are submitting a photograph, soundtrack, video or "written work." Your 1500-character (maximum, including spaces and punctuation) "written statement" (and, if submitting a 500-word [maximum] "written work") must be hand printed or typed on plain 8-1/2" x 11" paper (one side only; use additional paper if needed), on which you have also hand printed your complete name and complete address.
Mail your 1500-character (maximum, including spaces and punctuation) original "written statement" along with an original photograph OR soundtrack OR video OR 500-word (maximum) "written work," to: "Express How UC Success" Contest, P.O. Box 5780, Blair, NE 68009-5780, to be received by 4/21/10, when the contest ends. Additionally, you should include your complete name and complete address on your photograph (on back using a felt-tip pen or pre-printed label), soundtrack, video or "written work." Limit one mailed entry/submission per outer mailing envelope.
The following restrictions apply to mailed entries/submissions:
- Photographs must be unframed, unmounted, a black and white or color print on photo paper and be a minimum size of 4" x 6" and a maximum of 8" x 10". Slides, transparencies, professional or studio photos are not permitted.
- Soundtracks must be recorded on a disc that will play in a standard CD player.
- Videos must be recorded on a disc that will play in a standard DVD Player.
- Original "written work" must be typed or clearly hand printed on plain 8-1/2" x 11" paper and contain no more than 500 words (maximum).
- Original written "statement" must be typed or clearly hand printed on a separate plain 8-1/2" x 11" paper and contain no more than 1500 characters (maximum, including spaces and punctuation).
ENTRY/SUBMISSION REQUIREMENTS/RESTRICTIONS:
- ALL ENTRIES/SUBMISSIONS MUST BE YOUR OWN ORIGINAL UNPUBLISHED WORK AND CANNOT CONTAIN, INCORPORATE OR REFERENCE ANYTHING THAT IS OWNED BY ANY THIRD PARTY OR ENTITY OR THAT WOULD REQUIRE THE CONSENT OF ANY THIRD PARTY OR ENTITY IN ANY JURISDICTION. Any music, lyrics, sounds, props, or any other creative element used must be entrant's own original creative work and cannot have been previously published.
- Entries/submissions cannot contain any unacceptable clothing or adornments, or display any commercial/corporate advertising (including but not limited to corporate logos, brand names, trademarks, slogans, political, personal and religious statements) as determined by Sponsor, in its sole discretion.
- All entries/submissions become the sole property of Sponsor and Sponsor may alter entries/submissions submitted prior to any publication. Entries/submissions will not be returned. No correspondence will be acknowledged or entered into.
- Entries/submissions cannot contain any reference to specific treatment regimens, including, but not limited to, medicines, surgeries or recommendations for treatment or physicians.
- Video and soundtrack entries/submissions must be no longer than two minutes. If a video or soundtrack entry/submission is longer than two minutes, only the first two minutes will be judged during the Semi-Finalist Judging Phase (Rule #5). If a video or soundtrack entry/submission that is longer than two minutes and is determined to be a Semi-Finalist, only the first two minutes will be posted for online voting (Rule #6). Illegible or inaudible entries/submissions are void.
- "Written work" entries/submissions may contain a maximum of 500 words. Any "written work" entries/submissions that are longer than 500 words will be disqualified.
- "Written statements" may contain a maximum of 1500 characters (including spaces and punctuation). Any "written statements" that are longer than 1500 characters (including spaces and punctuation) will be disqualified.
- All entries/submissions must be in English.
- Video entries/submissions can be in color or black and white and may be enhanced with your own original graphics, special effects and/or animation, as applicable.
- Entries/submissions must not contain defamatory words/statements (including words or symbols that are widely considered offensive to individuals of any race, ethnicity, religion, sexual orientation, or socioeconomic group); they must not include threats to any person, place, business, or group; they must not disparage persons or organizations associated with the Sponsor; they must not invade privacy or other rights of any person, firm, entity, and they must not in any other way contain material that is unlawful, in violation of or contrary to all applicable federal, state, or local laws and regulations in any state where entry/submission is created. Entries/submissions must not contain material that promotes bigotry, racism, hatred or harm against any group or individual or promotes discrimination based on race, sex, religion, nationality, disability, sexual orientation or age. Entries/submissions may not contain material that is inappropriate, indecent (including but not limited to nudity or pornography), profane, obscene, hateful, tortuous, slanderous or libelous and may not in any way reference persons or organizations without their written permission. The entry/submission must not contain material that violates or infringes another’s rights, including but not limited to privacy, publicity or intellectual property rights, or that constitutes copyright infringement. Sponsor/judges reserve the right in their sole discretion to disqualify any entry/submission that, in their sole opinion, refers, depicts or in any way reflects negatively upon the Sponsor, the Contest or any other person or entity, or does not comply with these requirements or these Official Rules. Entries/submissions must be suitable for presentation in a public forum, in sole determination of judges and/or Sponsor.
- An entrant must obtain prior written permission from any person identified or featured in his/her entry/submission (or from parent/legal guardian if other person(s) identified or featured are considered minors in their state of residence), for Sponsor to use their likenesses for publicity including posting on Web site(s) and must make written copies of such permissions available to the Sponsor upon request. If entrant is considered a minor in his/her state of residence, entrant must receive permission from his/her parent/legal guardian to enter. PERSONS UNDER THE AGE OF 18 MAY NOT APPEAR IN A VIDEO OR PHOTO ENTRY/SUBMISSION.
- If submitting a video or photo, entrant must also receive prior written permission from the videographer or photographer of the entry/submission, if recorded or photographed by someone other than the entrant him/herself.
- Sponsor/judges reserve the right in their sole discretion to disqualify any entry/submission that, in their sole opinion, refers, depicts or in any way reflects negatively upon the Sponsor, the Contest or any other person or entity, or does not comply with these requirements or these Official Rules.
- Decisions of Sponsor/judges shall be final.
4. ELIGIBILITY:
This promotion is open only to residents of the 50 United States or the District of Columbia, 18 years of age or older at time of entry/submission, who are UC patients. Physicians are not eligible. Employees of Warner Chilcott, the Crohn's & Colitis Foundation of America, their affiliates, subsidiaries, advertising and promotion agencies and their immediate family members and/or those living in the same household of each are not eligible. All federal, state and local laws and regulations apply. Void where prohibited by law. All entries/submissions become the sole property of Sponsor. Sponsor reserves the right to: (a) use entrant's name and likeness and reproduce/distribute all entries/submissions in any and all media throughout the world in perpetuity, (b) alter, amend, edit or change entries/submissions prior to distribution and (c) use, distribute, reproduce, alter, amend and copyright entries/submissions for purposes of advertising and trade without further compensation, unless prohibited by law. The submitted "creative expression" must be outside of the entrant's primary profession, occupation or source of income. An entrant submitting a "creative expression" activity that is now or has been at any point within the past five years a majority of his/her total household income is not eligible.
5. SEMI-FINALIST JUDGING:
Judging to select the top ten entries/submissions will be conducted by D.L. Blair, Inc., an independent judging organization whose decisions are final on all matters relating to this contest, to determine the Semi-Finalists, based on the following criteria, as evidenced in both "A" and "B" components of entry/submission: Creativity of Expression — 30%; Originality of Expression — 30%; Inspirational Value of Expression — 40%. In the event of a tie, the tie will be broken based on the highest score for "Creativity of Expression." Winners will be notified by mail/phone.
6. ONLINE VOTING PROCEDURES:
Beginning at 12:00pm (noon) (ET) on 6/2/10 through 11:59:59pm (ET) on 6/16/10, up to 10 Semi-Finalist entries/submissions will be posted to the UC Success Fan page at www.facebook.com/ucsuccess. Visitors will be given the opportunity to view each of the Semi-Finalist entries/submissions and will be able to indicate a vote for their favorite Semi-Finalist entry/submission. Limit one vote per user id per day. At the end of the voting period, scores from the online votes will be combined with the judging scores from Rule #5, to determine five (5) Grand Prize winners. Each Semi-Finalist entry/submission’s percentage of the total votes will carry a 20% weight and the judging scores from the Semi-Finalist Judging (Rule #5), will carry an 80% weight in the determination of the winners. Winners will be notified by mail. In the event of a tie, tie will be broken based on the highest score from the Semi-Finalist Judging round.
7. AWARDS:
Each eligible entrant will receive a Recognition Award Package (limit one award package per person), consisting of:
- Certificate of appreciation to recognize his/her achievements and inspiration to the UC community.
- A copy of Celebrating UC Success, a booklet which features inspiring stories from other UC patients.
- A one-year membership to the Crohn's & Colitis Foundation of America (approx. retail value $30 each), which allows members to receive discounts on Crohn's & Colitis Foundation of America books and special programs; free participation in group support services hosted by local chapters; and invitations to educational and fund-raising events.
- A Crohn's & Colitis Take Steps palm card that offers information about how you can get involved in a Take Steps walk near you.
PRIZES and APPROXIMATE RETAIL VALUES:
Ten (10) Semi-Finalists will each receive a $50 gift card to the Crohn's & Colitis Foundation of America online store. Limit one Semi-Finalist prize per family/household.
Five (5) Grand Prize winners will each receive a 4-day/3-night trip for 2 to Seattle, Washington (7/16 & 7/19/10), and an opportunity to participate in the Crohn's & Colitis Foundation of America's Take Steps walk, on 7/18/10, including round-trip coach air transportation from major airport nearest winner's home, hotel accommodations (double occupancy) and a $250 check for winner (Approx. retail value: $2,500-$3,000 each). Winner and guest will be required to travel on dates specified by Sponsor. Dates of departure/return are subject to change. Actual retail value of the trip will depend upon winner's point of departure/return. Additionally, the Grand Prize winners will be recognized during Take Steps festivities as individuals who have expressed their UC experiences in a way that educates and inspires other UC patients and supporters. Grand Prize winners will be required to attend select festivities and Grand Prize winners' guest attendance is welcome, but optional. All winners are individually responsible for any taxes associated with prizes.
8. GENERAL RULES:
Potential Semi-Finalists (or parent/legal guardian if potential Semi-Finalist is deemed a minor in his/her state of residence) will be required to complete an Affidavit of Eligibility certifying that the entry/submission submitted is his/her own original work and he/she holds all rights to the entry/submission and all elements therein, that the entry/submission has not been previously published, has not won a previous prize or award and that he/she has followed the Official Rules of the Contest. Additionally, potential Semi-Finalists (or parent/legal guardian if potential Semi-Finalist is deemed a minor in his/her state of residence) will be required to complete a Release of Liability, Prize Acceptance Form and, if legally permissible, a Publicity Release. All Semi-Finalists will need to provide a diagnosis of UC that has been verified by a U.S.-licensed physician, to be eligible. If a Semi-Finalist entry/submission identifies or features persons other than the entrant, entrant must also submit a completed and signed Publicity Release Permission Form for each additional person depicted in the entrant's entry/submission. All documents must be completed and returned within 5 days of attempted delivery of same. Noncompliance within this time period or return of any prize/prize notification as undeliverable will result in disqualification and an alternate winner may be selected. Grand Prize Traveling companion (or parent/legal guardian if traveling companion is a minor) must also complete and return a Release of Liability within 5 days of winner notification. If a minor is selected as a traveling companion, winner must be his/her parent/legal guardian. All travelers must possess required travel documents (e.g., valid photo ID) prior to departure. No substitution or transfer of prize by winner permitted. Sponsor reserves the right to substitute prize of equal or greater value. All federal, state, and local taxes are the sole responsibility of winner. If an insufficient number of eligible entries/submissions are received to award the intended prizes, the remaining prize(s) will not be awarded. By accepting prize, winner(s) (or parent/legal guardian if winner is deemed a minor in his/her state of residence) agree to release and hold Sponsor, Facebook, their respective directors, officers, employees and assigns, harmless from and against any and all claims and liability arising out of use of prize. Winner(s) (or parent/legal guardian if winner is deemed a minor in his/her state of residence) assume all liability for any injury or damage caused, or claimed to be caused, by participation in this promotion or use or redemption of any prize. Acceptance of prize constitutes permission for the Sponsor and its agencies to use winner's name and/or likeness for purposes of advertising and trade without further compensation, unless prohibited by law. Entrants (or parent/legal guardian if entrant is deemed a minor in his/her state of residence) agree to release, discharge and hold harmless Sponsor, Facebook, its respective directors, officers, employees and assigns, harmless from and against any and all liability and damages. By participating in this promotion, entrants (or parent/legal guardian if entrant is deemed a minor in his/her state of residence) agree to be bound by the Official Rules and the judges' decisions which are final. In the event there is a discrepancy or inconsistency between disclosures or other statements contained in any contest materials and the terms and conditions of the Official Rules, the Official Rules shall prevail, govern and control. If an entrant submits more than the stated number of entries permitted, and/or if the Sponsor suspects that an entrant attempted to obtain additional entries by using multiple email addresses, registrations, identities or any other method, all entries submitted by the entrant will be declared null and void. Sponsor not responsible for lost, late, or misdirected entries/submissions, entries/submissions damaged during judging or quality of entries/submissions that are posted on the internet for voting (or advertising/trade purposes), or for any typographical or other error in the printing of the offer, administration of the contest, or announcement of the prizes. In no event will more than the stated number of prizes be awarded.

In the event a dispute arises regarding specific individual entitled to receive prize, entry/submission made by Internet will be declared made by the "authorized e-mail account holder.” “Authorized e-mail account holder" is defined as the natural person who is assigned to an e-mail address by an Internet access provider, online service provider or other organization (e.g., business, educational institution etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. Participant may be requested to provide Sponsor with proof that the participant is the authorized e-mail account holder of the e-mail address associated with the account/submission. Any damage made to the Web Site will be the responsibility of the authorized e-mail account holder of the e-mail address submitted at the time of entry/submission.
